ProCare Physical Therapy - Patient Information

To help you plan your day accordingly, each appointment at our clinic will be an hour long.

Appointments can be made Monday-Friday. If you are unable to reach us, please leave a voice mail and your call will be returned as soon as possible.

Office hours vary by therapist Monday-Friday.

Your First Appointment:
Please bring your insurance card, PT prescription, and completed new patient forms. **New patient forms can be downloaded and completed at your leisure or please arrive 15 minutes before your first scheduled appointment to complete them.**

Your therapist will need to access the injured area. Please wear accessible clothing depending on area to be treated:

Neck: Loose fitting shirt with large neck opening or sports bra
Back: Loose fitting pants and shirt and tennis shoes
Shoulder: Tank top or sports bra
Knee: Shorts or sweats that can be easily rolled up to expose the knee
Hips/Knees/Ankles: Wear tennis shoes
Elbow: Short sleeve shirt

Most insurance plans accepted - To ensure valid coverage, our staff will verify your specific benefits before your first visit. Co-pays will be collected at the time of service. Insurance claims will be submitted on your behalf and received payments posted appropriately. Should the insurance company send payment directly to you versus our office; it will be your responsibility to forward endorsed checks and explanation of benefits (EOB's) to us upon receipt.

Medical Records:
Records will only be released with your written consent. Record requests need to be submitted in writing. Please allow up to five business days for completion.
